Add Form 6 or Form 7 in a few steps
- Open the **Score Predictor** and choose **USMLE & NBME Clinical Shelf Exams**.
- Select **Clinical Shelf Score Prediction**, then **Family Medicine**.
- Add or confirm your target shelf exam date.
- Choose **+ Add CMS score**, then select **Family Medicine Form 6** or **Family Medicine Form 7**.
- Enter your **CMS Form EPC** and the date you took the assessment, then save.
Your saved score joins your CMS history and progress graph. With a target exam date set, the predictor uses your CMS results to update your estimated shelf EPC, giving more weight to recent assessments.
Use the EPC value for the **CMS Form EPC** field. If your official report presents a different assessment scale, consult its score interpretation guide for the corresponding approximate subject-exam score before entering it.
Forms 2 and 3 remain available
NBME retired **Family Medicine CMS Forms 2 and 3 on September 16, 2026**. Both remain available in CoreStepPrep, clearly marked as retired with the retirement date.
You can still enter a previous Form 2 or Form 3 result, keep your saved scores, and include them in your assessment history. Adding a new form does not remove your earlier work.
Forms **4 and 5** remain available, too. The predictor now supports Family Medicine Forms **2 through 7**, with release and retirement information shown where applicable.
